Many of us have our "wall of shame", or "shelf of shame", a good reminder of our vulnerability when turning.
I want to add that the most important points you raised in your safety suggestions would be the two mentioning starting the spindle with the speed turned all the way down. But in reality, all points are important. A mishap can happen with any one of them.
